The questionnaire consists of 4 sheets, contains 37 points, as well as a declaration that the applicant has provided authentic information, when applying for a multiple-entry visa, undertakes to issue medical insurance for each entry, agrees to the insertion and use of the specified data in the Visa Information System VIS.

P. 21-30 - general questions regarding the upcoming trip: the purpose of the visit, the number of entries (1. 2 or multiple), the country of the main stay, the country of entry to Schengen, the number of days of stay, previously issued Schengen visas for the last three years, the date of entry and departure This also includes the issue of fingerprinting (in Russia, biometrics on this moment  It is not charged, therefore in this paragraph it is necessary to put a dash).

P. 31 -33 - questions regarding the host country (in accordance with the purpose of the visit, the name and details of the hotel / inviting organization or personal data, address, telephone, email of the inviting person) and travel expenses are indicated.

Sections 34-35 are filled in if the applicant is a relative of an EU citizen.

P.36 indicates the date and place of filling in the form.

General rules for filling

Questionnaires are filled out on a computer, either by hand (in block letters, legible) or online, if applicable.

Application forms must be completed in English or in the language (s) of the country in which the consulate is applying.

Filling out the questionnaire in Russian is allowed by the consulates of Germany, Poland (in the case of submitting "paper" forms, and not online statements!), Estonia. Important!  In this paragraph 1-5 must be filled in Latin letters as in the passport, as well as in Latin letters filled in p. 31-32 - the name / address of the hotel or receiving organization or the name, surname, address of the inviting person (as in the invitation or confirmation of reservation) . The consulate of Finland also allows filling in paper forms in Russian, but only in Latin letters!

In the blank columns, a dash is put (for electronic questionnaires, symbols are used in accordance with the instructions).

A.1 - 5 - the name and surname, place of birth are written as it is indicated in the passport. If the city / town of birth is not specified in the passport, you still need to enter it.

P. 6-7 - country of birth, citizenship at birth: most of the Schengen countries require to write the USSR for persons born before 1991.

Exceptions:

  • France - everywhere the name of the country is indicated at the moment,
  • Czech Republic - persons born before 1991 should write in the column "country of birth" -RUS, in the column "citizenship at birth" - SUN),
  • Sweden - persons born before 1991 must write in the “country of birth” column - USSR or RUSSIA, based on which of these names is indicated in their international passports.

P. 17 - The address of actual residence and the telephone number, by which you can always contact the applicant (preferably mobile), is indicated.

P. 19 - the profession / position is indicated. If the applicant is an unemployed / student / pensioner, this clause accordingly indicates: UNEMPLOYED / STUDENT / RETIRED.

Item 20 - information about the employer is indicated (name of the organization, address, telephone number, e-mail). Pensioners, unemployed and children of preschool age do not fill this item, students and schoolchildren write the name and address of the educational institution.

P. 22 - indicates the main stay. It must correspond to the consulate to which the application is submitted. If several countries are planned to visit during the trip, the main one is the one in which the most days are planned. If the number of days is the same everywhere, the main country is recognized, the border of which will be the first entry into Schengen.

Item 23 - the country is indicated, the border of which will be the first entry into Schengen.

Important!  When filling out an electronic questionnaire for a visa to Portugal, Germany cannot be chosen as the country of first entry (a technical feature of filling in a questionnaire), even if it corresponds to tickets. In this case, Portugal is chosen.

P. 24 - the multiplicity of the visa is selected (single, double, multiple).

P. 25 - indicate the number of days to be spent on the territory of Schengen (for multi-vise, the total number of days for the entire period of validity of the visa is indicated, for annual and more days - "90").

P.26 - indicates the duration of the Schengen visas issued in the last 3 years. If all visas cannot be accommodated in this column, the last one or two is indicated. When completing the online application form, this item indicates the latest visa, as a rule, there is no larger number.

P. 29 - 30 - indicate the date of entry and the date of departure from Schengen. If a multiple entry visa is requested, the full visa validity period is indicated, starting from the date of the first entry.

For example, when applying for an annual visa, if in paragraph 29 the date of the first entry is 10.07.2014, then in paragraph 10. The date of departure is 09.07.2015.

P. 31 - the data of the receiving party (individual) or the name of the hotel, address, e-mail, telephone and fax are indicated. If there are several hotels, you should indicate one of them (the rest of the reservation is attached in the general set of documents).

P. 32 - filled only by those who request business visa, or goes at the invitation of the organization.

P. 33 - should indicate the person paying for the trip. For minors, the name and surname of the parents is indicated. For non-working persons and pensioners - the name and surname of the sponsor. In case the trip is paid by the inviting person, his details are indicated.

Item 36 - the date and place of filling in the questionnaire (the place means the place of residence of the applicant)

P. 37 - applicant's signature. For minors, the signature is put by the parent or legal guardian. The following Czech signature requirements apply to the Czech Consulate:

In the case of a child under 15 years of age, the questionnaire is signed by one of the parents or guardians, or another legal representative. If the child’s age is from 15 to 17 years old (inclusive), then the form must be signed by one of the parents and the child.

In the consulate of Austria, the requirements are more stringent:

Minor children under 14 must have 2 signatures: father and mother. Minor children from 14 to 18 must have 3 signatures: father, mother, child.

Also, signatures are put in the text of the declaration (clause on confirmation of reliable information, clause on insurance) and under the declaration.

Additional rules for online questionnaires

Electronic questionnaires are filled in accordance with the general rules, however, there are a number of nuances.

In most cases, before filling out the electronic questionnaire, you need to go through a simple registration (the exception is the submission of an application through the EC of Switzerland) - that is specify the email address, first name and surname, as well as select the city in which the consulate is located.

After the information is entered (the system automatically points to incorrectly filled or missing items, if any), you need to choose what to do with the statement. Send it or leave it for possible adjustments.

If the "send" button is pressed, the application automatically goes to the database of the selected consulate. A form will be created for the applicant, which contains all the specified information, as well as a bar code or a bar code. The blank of the questionnaire must be printed (together with the code - as a rule, it goes as an additional, fifth page), sign and attach to the main package of documents.

If, after filling out and sending the questionnaire, an error is found in the information, it is necessary to fill out a new questionnaire and print the form with the code again. Corrections to individual paragraphs without the need to re-fill the entire application allows the consulate of Finland, however, in this case you will need to print a new form with a new code.

Some consulates, for example, Slovakia, Switzerland, Portugal, Poland, and Finland, in addition to filling out the questionnaire themselves, also provide the possibility of setting an appointment time online. Usually this function is offered to the applicant immediately after the application form has been successfully completed and saved.
